Speaking of bloody edginess. The official board game adaptation by Milton Bradley features what I believe was the intended design for him.

Most fanart depictions go over-the-top in bloody edginess, which kinda misses the point.

I mean a giant moving smiley face in itself is pretty scary.

Funny thing is, they wanted to make Evil Otto something more intimidating.
But then they thought it would be funnier if he stayed as a smiley face.

Honestly, the sprites here would work just as well if they were aligned against the left instead of being put in the direct center.
You could put the text into two rows per colum to reduce “negative space” if you’re that concerned over it.
